To start your journey to weight-loss, you MUST complete your Patient Information Packet in order to make your first appointment. Your information is required by most insurance companies to qualify for surgery.

Important notes to remember in completing your information on the packet:

• Be as thorough and accurate as possible.
• Allow approximately 30 minutes to complete. You don’t have to complete all at one time; you can save your work and finish later.
• Have all of your medications available for listing.
• The Patient Information Packet must be completed on a laptop or desktop computer (cannot be done on an iphone, ipad, kindle or other PDA's).
